CONSENT FORM MANDATORY FOR IN PERSON LEARNING

In order for your child to return to in-person learning, you MUST submit a consent form for in-school COVID-19 testing. If you have already submitted consent, we encourage you to do so again to ensure your student has the latest form on file. You can update your consent form on your child's www.mystudent.nyc account .  
Students who need a medical exemption due to a health condition, and students with disabilities who cannot be safely tested in school due to the nature of their disability, can submit a separate request for an exemption.

GET TESTED
COVID19 Testing is being prioritized for all NYC schools students (3K - Grade 12) and employees at 22 NYC Health + Hospitals testing sites across the city during the 2020-2021 school year. Click to find the list of DOE-priority free testing sites & Hours. Test results will be available within 48 hours after testing.
